If you complete a form on this site we collect what you type into it. Depending on the form that includes your name, mobile number, email address, your pet’s name, and whatever you tell us about why you want to be seen — which may include information about your pet’s health.

If you arrived from an online advertisement we record the advertising click identifier so we can tell which advertisements bring people to us.

Form submissions are retained for as long as we have a business or clinical reason to keep them. Veterinary medical records are retained as required by Florida law.
